Reconstruction after gynaecological / colorectal / hydradenitis / pilonidal disease surgery
Excision of gynaecological or low rectal cancers may require removal of significant tissue within the perineum or walls of the involved structures. As Reconstructive surgeons we are often called by our colleagues to reconstruct such defects. Hydradenitis and pilonidal disease are benign but often relentless inflammatory/septic conditions of the perineal/gluteal cleft area as well as other areas of sweat gland baring which often require radical debridement. Amongst our armamentarium are:
- Laying open of sepsis and healing by secondary intention
- Excision and split skin grafting
- Local flaps based on the external pudendal system (Lotus petal flaps)
- Gracilis mycoutaneous flaps
- Gluteal myocutaneous flaps
- Lumbosacral or gluteal fasciocutaneous perforator flaps.
- Vertical rectus abdominis flaps passed through he open pelvis
